Coming Home from the Hospital or Rehab?
Many families first hear about Home Help when a loved one is about to come home from a hospital or a short-term rehab stay. Eaton County families are often discharged from UM Health-Sparrow Eaton in Charlotte, UM Health-Sparrow Lansing, McLaren Greater Lansing, Mary Free Bed at UM Health-Sparrow, or Eaton County Health & Rehabilitation Services in Charlotte.
Call us before discharge day. We can start your caregiver paperwork while your loved one is still in the hospital. That way you're further along when they get home. It also helps to tell the hospital social worker or case manager that you'd like to be your loved one's paid Home Help caregiver.
Do You Qualify?
You may qualify if you can say yes to these:
My loved one has full Michigan Medicaid (or I'd like help understanding how to apply)
They need hands-on help with daily tasks like bathing, dressing, meals or medications
They live in Michigan, in their own home or a family home (not a nursing home)
I'm 18 or older and can pass a background check
I'm not their spouse, and they're not my child under 18
Not sure about one? Apply anyway. We'll help you figure it out.

Questions from Eaton County Families
My adult son has a disability. Can I be paid to care for him?

Now. We can start your caregiver paperwork before he comes home, so you're further along by discharge day.
We live in Grand Ledge. Can we meet in person?
Yes. Our Okemos office is a short drive away, and after screening a Genesis team member can visit you at home.
What if my loved one doesn't have Medicaid yet?
